CentraCare Health Foundation/CentraCare Clinics has received a grant from the UCare Fund to support the extension of a six-month pilot project which added a Somali community health worker to the primary care team at CentraCare Clinic – Women & Children.
The UCare Fund awarded the $32,950 grant, which will allow the Somali worker to provide bilingual and culturally competent behavioral and developmental screenings for Somali children ages 0-5, and health education to prenatal mothers and families, it was announced Thursday.
The UCare Fund is a community-directed initiative of UCare, the fourth-largest health plan in Minnesota.
